Policy Statement

(Revised 12/1/2011)

Payment for Services Rendered
Payment for services rendered will be due within 30 days of receipt of the invoice, which will be emailed to you. We now have online payment options available for your convenience. Those failing to pay on time may be subject to late fees and/or cancellation of future appointments. If you have any questions regarding your bill, please email Jenn Drew at billing@angelfishtherapy.com.

Cancellation Policy
Notification of cancellation for any appointment must occur at least 24 hours before the appointment. Those failing to do so will be charged a $75 fee. If you need to cancel, please contact the therapist directly, and also email us at info@angelfishtherapy.com (or call us at 203-545-0024). In cases of extreme circumstances, please contact us to discuss a discount or waiver for this fee.

“Code Brown” Policy
The YMCA and hotel pools that we utilize for services all have strict “poop in the pool” policies, and will charge a fee to cover maintenance costs and shut-down of the pool. When this occurs, these costs will be passed on to the family responsible. To prevent this from happening, if your child has any risk of this happening, please ensure that he/she is wearing a swim diaper covered with rubber pants.

Safety and Siblings
For safety’s sake, any children — including siblings — present at the pools should always be closely supervised by parents/guardians, particularly when no lifeguard is present. Remember, pool decks can be slippery, making running in pool areas dangerous. Except in cases where an Angelfish therapist or instructor gives special permission, siblings should not use the pools, and should try not to interfere in any way with ongoing sessions .

Towels and Changing Areas
Please bring a towel from home for your child. Use of changing areas is fine, but use of hotel, YMCA, or JCC towels is not permitted.

Collaboration with Other Professionals
As always, we are happy to collaborate with any other professionals involved in your child’s treatment goals and objectives. Feel free to talk to us about it.

Insurance
If you require letters of justification for your insurance company, just let us know. While we’re considered an out-of-network provider, and can’t process claims directly, we do want to assist you in any way we can to get reimbursement for your child’s treatment.
